Post menopausal bleeding

Hi there everyone. I wonder if anybody could relate to my experience at all? I have been on HRT Utrogestan, and Evorel 75 patches for approximately 2years. I was on Evorel Conti 50 for about four years before that. I was changed over to the Utrogestan and Evorel 75 when I was 55, and the doctor considered that I was probably through my menopause ( how do you ever know if you are on hrt?). No problems on Utrogesten, and Evorel 75. I was getting a little bit of pain in my lower left abdomen, near my left ovary, and in December I went for an ultrasound and a small fibroid was detected. Last Tuesday, I started with some brown bloody discharge, and then by Thursday this had increased to a period like flow which I still have today.

I have just spoken to my GP who gave me the choice of coming off my HRT immediately for six weeks and seeing if the bleeding stopped, or carrying on with my HRT and being referred for a routine gyne appointment which will maybe months.

I am quite shocked I’m very upset by all this. I have decided to come off my HRT and pray that the bleeding stops.

I know that nobody can give me any reassurance, so I hope it’s okay to just post and ask if anyone has had any similar experiences?

Please be gentle, I’m not in the best of places at the moment, thank you

Please take the GP advice and if it continues longer than a period get back to the GP.
I had similar on HRT, I got a brown discharge too. GP on the ball got me a Ultra sound and Doctor did a Biopsy while I was there.
It was a problem, and I was treated quickly.

Thank you for taking the time to reply. So sorry that was your experience. I’m a bit all over the place mentally with all this to be honest.

And thank you for being so sensitive in your choice of words - it’s much appreciated ?

I had the same, 3 years after periods had stopped, I had to stop HRT which stopped the bleeding. A couple of months later I had a scan, and everything was fine. It is quite common apparently. However, still, follow up with the doctor even if the bleeding stops.

Try to remember it is more likely to be nothing and if it were cancer it is treatable.

break through bleeding is common ,it might just be you need your HRT dose tweaked .I'd sit it out for a wee while and if you think its getting worse get back in touch with your GP.
My GP sent me to the gyn clinic last week for spotting when she noticed it on my smear test.I did tell her it wasn't anything but she wouldn't listen. That meant I took up a long appointment for internal and external mears and blood tests that showed ...exactly nothing
I always believe you know your own body best and bleeding is n't always something serious.
Good luck with it but dont panic ,hope it all goes well

Thank you Elizabeth and Paddyann. Thank you both for your help, and for sharing your experiences. I am really concerned about just stopping my HRT immediately, but the GP said they were the guidelines. I cannot imagine how stopping so suddenly it’s going to impact me, as I already struggle with anxiety and depression.

I think that I will contact my GP again tomorrow, and have another chat with her. It seems a bit of a cruel choice to make - immediately stopping my HRT and waiting six weeks, or staying on my HRT and waiting for an appointment which may be months and months.

I really appreciate your replies, they have helped settle my mind a little.

It can feel terrifying and isolating trying to work out the best thing to do when you don’t really know what you are dealing with.

Dont stop your HRT until you see a specialist ,ask for an appointment at the menopause clinic or a gyn clinic .GP's often know next to nothing about HRT and Menopause .If you stop it now they wont let you go back on it so try to work through it .Phone your GP and ask for a referral asap ,if you are worried .If they say theres a long wait phone the Gyn clinic direct ,explain whats going on and ask to be seen

Thank you Paddyann. I’m going to put another patch on tonight ( I took the one I had on earlier off). I will ring the GP tomorrow, and ask to be referred to the Gyne team. I was seen by the Gayner consultant about a year ago for thrush/ dryness etc. she is the consultant who put me onto the Evorel 75 and Utrogestan 2 years ago. Thank you all for your help. I will let you know what happens.

I have been on HRT for a number of years and like you would not be able to cope without it. The side effects of suddenly stopping it are horrible.

I have had exactly the same thing happening to me a few weeks ago, with all the symptoms I used to have with a period: sore breasts, feeling faint and a dull pain at the bottom of my stomach and bleeding.
I had a telephone appointment with the GP the next day and he immediately referred me to the Gynaecology department at my local hospital. I got an appointment within 2 weeks.

I was given an internal scan of my uterus and ovaries and given the results 15 minutes later. Then an external examination on my request. Thankfully everything was fine and just a very small fibroid was detected.

The GP and nursing sister did not think it was necessary for me to come off HRT and advised me to contact my GP again if I had a recurrence of the bleeding.

I had considered going privately if I could not get an appointment quickly, but it was not necessary.

Push for an immediate referral as paddyann54 has suggested. You should not have to wait for a routine Gyny appointment in this situation.
Good luck

Hi everyone, I hope that you are all okay this morning. Thank you again for your help last night. I have spoken to my local Thankyiu Bakingmad for sharing your experience, and I’m so glad that you are ok. I related a lot to your post. I have made the decision to stay on my hrt, I’m thinking at this point it is the best thing for me to do mental/physical health wise. I have contacted my local Gyne team independently this morning, after a very agitated night, and they were just brilliant. I have a telephone appointment booked for Tuesday morning, with the menopause consultant that I have seen before. I am still feeling very scared and fragile, but with everyone’s encouragement I have speeded things up for myself. I felt totally abandoned by my GP yesterday, and left with impossible choices. The choices may still be the same, but I feel that I will be given a lot more support with them through the Gyne team. I will of course let you know what happens, and thank you all again for being there for me.

I had post menopausal bleeding and was told it always warrants a 2 week hospital appointment. I also had heavy discharge that caused irritation but despite an internal ultrasound and smear test nothing showed up. My GP thinks it’s being caused by vaginal atrophy and prescribed topical estrogen (I was already taking tablets but had reduced the dose). The bleeding has reduced as has the discharge. I didn’t even know that vaginal atrophy could cause this, but seemingly it does.

Hope this doesn’t frighten you but insist on a scan/ tests to make sure your cervix and womb are ok. My results were pre cancerous - had to go in for laser treatment under general anaesthetic. HRT had to stop immediately - they said it was the cause.
Scenario was similar- feeling good on HRT but started bleeding/ spotting. Referral was quick (a few years ago). Maybe consider a private appointment if NHS can’t react quickly
Let us know how you get on.

I think the Gp is wrong to say 'all or nothing'. I had breakthrough bleeding when I started HRT, although it was bio identical prescription privately through the Gluck clinic, and they just reduced the estrogen a bit and that sorted it.
It's ridiculous to think that everyone needs the same dose.
I would say read up a bit before your appointment, watch the documentary on 4 and then you'll feel more prepared before your appointment.

I had the bleeding whilst on HRT and was referred within two weeks for a gyny test using an ultrasound scan. A small polyps was found and I was operated on within another week (day op)… a biopsy was done as a matter of routine and no issues.

2 years on I am due for a routine scan and less scared than I was before.
The NHS knew of my unfortunate experience as a child and took great care with the examination.

I hope you get this sorted quickly!

I was also referred on a two weeks basis when I experienced a little bleeding, I'd been on Kliovance for a few years. I was sure in my own mind I just needed the HRT tweaking and it scared me silly. In the end I was pleased this had happened as the gynaecologist checked me over and agreed, took a tiny snip from the uterine lining very gently which ruled out anything nasty. He recommended Oestrogel and a Mirena coil as being the best and safest way forward - for life if I wanted. I'm temporarily on patches at the moment due to the oestrogel shortage but relieved and happy to be sorted.

Same thing happened to me. Turned out to be vaginal atrophy. The scan terrified me at the time but it was reassuring to have such a thorough check up.

Try not to worry but get yourself tested as I had post-menopausal bleeding and it turned out to be endometrial cancer. I was lucky in that it was found early, the treatment was started within a few weeks, and I've been clear for over ten years.
