Good evening
Can anyone out there offer any advice on how I go about getting an assessment for my dad for continuing health care. My social worker doesn’t seem to want the help. My dad is in a care home and gets 24 hour care.

You need to request the checklist is done first...do they have district nurseComing in? They can so the checklist....but do your research first...its very difficult to get ....your needs needs to be complex ...and be health needs as opposed to care needs

I persuaded a nurse to do the checklist with me but she wasn’t very helpful and didn’t know much about it. She down rated all the scores I gave for my Mum despite only seeing her once!

Our Social Services assessed my Mum every year when she was in the care home even though she was self-funding.

That will a social care assessment....completely separate to CHC funding which is NHS
