Want to write a living will

I have written down info for my DDs about my care wishes in the event of becoming incapacitated and they know where it is. They already have P of A for both care and finances. I trust them to do the right thing and make sure others do as they were involved in the difficult care decisions over my late OH at the end of his life and stood up for him.

What we’ve done is to add a paragraph to our Health and Welfare Powers of Attorney, saying roughly,

‘If I should develop dementia, or any other condition where I am unable both to care for myself, and speak - with full mental capacity for myself - then I emphatically do not want any life-saving or life-prolonging treatment. I ask for palliative care only.’

When I did my LPA I got my solicitor to add what I wanted to happen if I became seriously ill. I left the wording up to her as I didn't know how to word it. The wording she used was brilliant but didn't go far enough for me so saw my GP about adding a DNR to my notes. I had to fill in a form which will be looked at every year. My LPA was also scanned into my notes as that makes it easier to prove what I want and not just my family saying it.

Has anyone done this? What did you include and who has a copy? I’ll keep a copy for myself and give one to my daughter but was thinking I could send one to my GP and get it scanned onto my medical record 🙏

Its very specific what it can be used for ....look at some of the online guidance .....and most definitely provide the gp with a copy

My old neighbour got DH and I to witness and sign a living will regarding how he wanted to be treated should he become incapacitated.

I believe a living will is only for advance decisions about medical treatments you may choose or refuse in the future. It must be signed in front of a witness if the choice is to refuse life-sustaining treatments, and given to your GP.
It can also be done in conjunction with a LPA.
