Having been a teacher and worked with small children both here and abroad, I was quite used to all sorts of different words, or none at all , but for me the important thing was to know what a child would say when they needed to use the lavatory. Having mixed nationalities added to the problems sometimes and so I wanted parents to let me know what they said, but once you had larger groups of children it was much better when you could all have an expression that was easily understood by everyone, which avoided accidents and upsets. So it often ended up with something like "does anyone need the lavatory/toilet now?" and children were able to connect this to their own particular choice of words within their family. But that was for more general use rather than within a family I know.
If all your family use the same words it is easy to follow suit, but you do have a problem when each grandchild has their own words , so you have different things to remember for each one. I think if it was me I would make a note of each ones choice of words but maybe decide on whatever words seems natural and easy for you to use. Then you could use that word, explaining to each grandchild that you called it vagina or fanny or whatever and that it was the same thing as the word they used. That way with more than one grandchild you have your own word that they can use with you, and you wont get muddled.
