Please be careful about taking ibuprofen on a regular basis without taking omeprazole, too.

That sounds awful. To get suffering fo so long and it seems no follow up care?

I suffer from internal haemorrhoids, which play up occasionally.
Not so long ago I had awful stomach pain/ cramps followed by severe bleeding - literally flooded the toilet pan. But it stopped within a couple of minutes.
Saw GP in the morning. She examined me but found nothing sinister, thank Heaven.
I know it’s very scary
Btw I think I know what BM stands for but I have only heard of GTN spray being used for heart symptoms. I might be wrong Apologies.

it needs to be removed surgically probably, but injections might work.

Have you seen a specialist?

Family member suffered similarly. Ask about diltiazem, think it's off label, Botox, Scheriproct (this shrunk my chronic haemorrhoids to a manageable state)but ultimately I think you'll need an operation.

I am still struggling with the daily constant pain.

The GTN ointment is a muscle relaxant that had initially eased the edge off the pain, but that has since stopped working.

I aim to apply (unfortunately having to insert using a gloved finger)! x2 a day but now it hurts so much to insert, as it feels like my bottom is tearing/stings & burns so much each time, triggering the on going burn/pain.

It feels like have at least 10 sharp paper cuts on, in & up my bottom!

The GP is going to request advice & guidance with a specialist as she feels awaiting a referral would take longer.

This situation is really getting me down.

I just don’t understand, why or what the issue is!
Is it an undiagnosed fissure? Or is this usual for haemorrhoids?

And why has it remained unresolved after 6 months??

I really need to get back to work but still feel unable to focus due to the ongoing pain!

Any advice or suggestions are welcomed 🙏🏻🤞🏼🙏🏻

Sympathies OP any pain or problems in that area are very upsetting and can really get you down.

Diltiazen helped me. I was in agony. I am going to ask GP about it again as it came back a couple of days ago. It really is painful. I just take paracetamol, I don't get in with Ibruproprem.
A couple if years ago I went to A&E as it got so painful. I got diltiazen then.

I've been experiencing chronic pain since March. Only had digital examinations and told it's was a thrombosis haemorrhoid!

yet reading up my symptoms mirror a possible fissure.

Agonising pain 8-12 hours after a BM.
My internal anal sphincter constantly spasms and that's what causes the pain.

It has taken a GTN ointment to relax muscles & it eases up after 1 week of using it but then when I stop it symptoms flare back up & the pain & intermittent bleeding returns,

(Unsure why I've developed these symptoms as I’ve never had troubles in these regions before)

I've been off work since May as the symptoms & pain has been too much to bear! (Feeling embarrassed and pathetic for not being fit to work due to bloody piles!!) but seriously the pain!!!

On the worst days, I've been bridging paracetamol & ibuprofen every 2 hours, days on end to take the edge off the pain.

I'm not even constipated & I continue to have 1 BM daily. Yet remain in constant pain for hours & hours after!

I'm at a loss as I don't know how or what to do to????

I have a thrombosed haemorrhoid and no spasmodic nor constant pain at any time.

Have you consulted a GP recently or asked for a second opinion?

I am not a professional but I think a digital is not enough and you need an examination under light anaesthetic or analgesic of a sort where the doc can literally see what is going on.

You ask for advice. I can recommend a sitz bath -type bidet with warm to hot water. It may take some time for a plumber to plumb it in, although it is usually a simple job a DIY handiman can do .My son plumbed in my bidet,

However you can buy a portable sit-in bidet from Amazon and get it the next day. If you do, get a deep one with an overflow hole in it. The plastic bidet basin is made to fit inside the lavatory pan. You fill it from a jug or hose pipe attached to the bath or basin tap.

Try adding a very little salt to the water no more than blood saltiness!

Your description of sharp paper cuts triggered my memories of experiencing a polyp. It felt as though I had a holly leaf inside. I had a colonoscopy and they removed a polyp.

That sounds awful Florence. I had a haemorrhoid a while back and that was a horrible experience so I can only imagine how much worse this must be for you. You have my sympathy. I agree with Caleo that some sort of salt water bidet may help. You may be already doing this but I would be pecking my GP’s head trying to get further investigation and / or getting a second opinion. I do hope you heal soon.

How awful for you. I know little about piles myself but I wonder if you're right about a fissure. It is possible for something like a fish bone to cause a tear in the bowel. Or perhaps you're eating something which causes inflammation. If I eat a lot of nuts, I get severe pain in the back passage, but it soon wears off.

You would expect that the diagnosis is from visibly seeing the problem and I know that it can be very painful. I believe it can arise from stress as well as constipation etc. I’m surprised you haven’t been prescribed pessaries but I think you can buy them over the counter anyway. They ease the pain specially after a BM. There is a surgical option where I think they inject it with something….you might want to ask your doctor. Good luck.

Thank you. Yes but it is important the saltiness should be very slight. Taste it and it should be about the same as blood or less. Too much salt burns .

I have polyps which can be uncomfortable and bleed which is a nuisance. Depending on the situation I wear pads of differing sizes which makes me more comfortable but it can be unpleasant. I have every sympathy for you FlorenceB19
Your situation sounds very uncomfortable. Have you been to a specialist rather than a GP? It would be costly but could be the answer

Unfortunately, I am still having issues with intermittent bleeding & intense pain especially with & after a daily BM.

Thankfully, more recently the pain appears to not be has severe as the initial 5.5 months. As in the continuous intense pain but still very painful.

I’m unsure if it’s a sign things are settling or if it’s the fact that I’ve got a accustomed to the pain levels.

Thankfully, I now have a colorectal surgery appointment at the local hospital booked for 1st October.

I have no idea at this stage what the appointment entail’s, yet either way I’m hoping to get to the bottom of this issue (pun intended)!

My mood has got really low, I feel like my life has been placed on hold for 6+ months & still counting. Each day has been a waiting game for everything to go back to normal & pain free.

I will add that , at the same time of these symptoms kicking off, I had also been diagnosed with over active muscles & in turn developed Hip bursitis and gluteal tendinitis.

I’m unsure if these symptoms are related and if/or they have triggered each other condition.

I am also pending surgery to remove an ovary alongside a Dermoid cyst. Again could this be related or triggered symptoms?

I am 4 years post menopause.

Has anyone else experienced anything similar?

Years ago I suffered with this and I really feel for you.
My advice is get hold of all the pain relief you are allowed and whatever you do post surgery or banding or whatever is done to help you with this. Make certain the pain relief is continuing and don’t try to be brave.
The most distressing and painful thing to endure.

Just a little update. Seen colorectal consultant and he confirmed anal fissures & possible hypertonic pelvic floor. Will try different cream diltiazem for 6 weeks.

Also referral for a mini scope camera to ensure nothing else being missed. Hoping to resolve the pain & spasms.

Still unsure what’s causing my pelvic muscles, thigh & glutes to spasm.

Just want relief & my old life back!

I read somewhere, many moons ago, that there is a concentration of nerves in the anal area and this is why a fissure (or other problems) are so painful; some sufferers saying that at times a fissure could be as painful as labour. No wonder you needed constant pain relief. Glad to hear that you have finally got a diagnosis and hopefully the treatment given will sort out your problem.

Thank you….. I would agree there although, it literally feels like I’ve given birth daily for the past 6 months 😫🫣😵‍💫 which probably explains the impact on my low mood.
