I think that some sections of the Press have had a tradition of picking on the younger female members of the RF. I remember when it was Princess Anne, then of course it was Diana. Gossip pushes up sales.

The press and media are bullies and they like a victim.
Young females - be they members of the RF, celebrities, elected representatives, or engaged in sport - have always been targets for some sections of the media, I think.
It's open to debate, but I believe this particular section of the media will frequently exploit and capitalise on the 'rise' and popularity of a chosen woman who is currently high-profile, and then enthusiastically report on her gradual and sometimes inevitable downfall. Of course, these 'targets' sometimes court the very media that is going to delight in her rise and fall.
The worst thing is that said media's reportage is done in a seemingly innocuous way, as if it is just an impartial observer, but this is IMO simply a devious tactic that allows them to take full advantage of the woman's vulnerability (and, sometimes, stupidity it has to be said).
I wish women would realise that certain media is male-centric and misogynistic and that very few women will be celebrated for their achievements, only their appearance. Why else are celebrities, like Dame Helen Mirren, portrayed under the caption "she's still got it"? If she hadn't (still 'got it'), her accomplishments would go completely unnoticed.

Of course. The media have the unwritten contract on what to report on the royals, and undoubtedly know exactly what is going on. So, in order to ‘thrive’ ( i.e. sell papers or attract viewers), they report what people are saying online, all the while prefacing the stuff with the fact that it’s from the internet.

Catherine is recovering from major surgery some of us have been there so know just how long that takes. I’m just very sorry that she has been the subject of speculation when she is probably feeling quite low in herself.
Where is the support for her from those that are looking out for her. The silence from that quarter is deafening.

If they said that Catherine, or the Princess of Wales thanks people for their concern but wishes the details of her operation and her recovery to remain private, would the media and the great British public accept that?

The publication of the photo was meant to stop the ongoing and intrusive speculation. It wouldn’t have worked even if it hadn’t been edited.
Yes, someone could have advised her against offering it, which she did to try and quell speculation.
How would you protect her from the constant speculation, whether we refer to the photo or her health?
Tell her not to read magazines, watch the news, and try not to think about all those talking about her behind her back?
I have no idea what else could be done apart from asking the media, newspapers, online news, TV, and social media to stop speculating and publishing the speculation.

I think as part of a team as she is you’d expect the team to be responsible but in this case it seems to be all down to Catherine.
Seems a lonely place to be whilst recovering from major surgery. I’d have expected much more from my team if I was in that position.
I am sure she is having much support from her family and friends and from William’s family and friends.

You don't need to be an expert (expert in what?) to understand that conspiracy theorists will conspire to theorise regardless of how much information they are given on her recovery. Whatever detail is released, they will speculate that more is being hidden from them.
Sometimes, when people have abdominal surgery, its effects leave you in a condition which you'd rather keep private. Maybe Catherine (not "Kate") would prefer to keep those details to herself?
Why should she pander to the "theorists" just to sate their idle curiosity?
